* Uses `inline-table` on the input group
* Nukes the widths to `width: auto`—without this, the parent input
group doesn’t size correctly and functions as `display: table;` or
`block`

9dc796155c Fixes #12722: Fixes up responsive tables in print 
Safari renders this fine for one reason or another, but Chrome still renders the media query styles to make tables responsive. This change scopes them to screen devices only, so printing looks boss everywhere.

The order now used everywhere is the one most frequently found in the repo before:

2979e4bcea Add new form feedback states, clean up existing form styles 
New stuff:

* Adds `.has-feedback` and `.form-control-feedback` to show icon-based
form feedback to users. Fixes #11836.
* Removes the `width: 1em` on `.glyphicon` because it makes it
impractical to resize the width of all icons, even if they’re not
“empty” (e.g., a simple `.glyphicon { width: 30px; }` wouldn’t work,
nor would a class preceeded by a parent class.

Clean up:

* Sets `width: auto;` on all `.form-control`s within inline forms.
* Removes all the inline `select` menu `width` styles because all
inputs are now `auto`.
* Update form states docs to separate out sections better.

11c6ab767c Fixes #10370: Improved and deprecated dropdown menu alignment options 
* Removes an old pair of selectors that didn’t properly target the
right-aligned navbar alignment of dropdown menus.
* Deprecates the `.pull-right` alignment in favor of a more specific
and unique class name.
* Adds `.dropdown-menu-right` as the new alignment class. This is then
mixin-ed into the `.navbar-right.navbar-nav` dropdown menus for
auto-alignment.
* To override that auto-alignment, use `.dropdown-menu-left` as needed.

Since we’re switching to `grunt-contrib-less`, we can take advantage of
newer LESS features than what RECESS supported. Included in that is the
ability to `:extend`, and not only that, but `:extend(.mixin-name
all)`. By doing so, we remove duplicate CSS for all our elements that
were being clearfix-ed.

Fixes #8947, #8968, #8991, #9257, #9268, #9291, #9430, #9604, #9686,
#9929, #10731, #10793, #11305, #11498, #11533, #11570, #11604, #11652.

5a0365524d Rework navbar padding and margins for right aligned last-child elements 
This slight reworking of the navbar spacing was brought about by a
problem noted in #11530: use of `.navbar-form.navbar-right` leaves an
extra 15px of space on the right.

Here’s what’s new:

* Removed the `padding` on `.navbar-collapse` within static top, fixed
top, and fixed bottom navbars. Meaning, it’s still there for the
default navbar. This was the root cause of the extra 15px padding for
navbar forms.
* As part of this, I also replaced the existing negative margin
overrides for last-child elements with more contextual ones within
their sub-components.

Overall not too different, but should be more effective now and easier
to work with now.

441f9d6717 Support .h1 in place of h1 inside .jumbotron 
As in #7284, it can often be useful to style text like a heading in
places where using the heading tag would be either semantically
incorrect, less accessible, or otherwise invalid.

Support this in .jumbotron as in the rest of the document by allowing
elements with class .h1 to stand in for the h1 element.

a78c8d9c04 Newfangled input and button group sizing (fixes #9295) 
Instead of applying `.btn-` or `.input-` to individual elements in
button and input groups, we now have new sizing classes to keep your
markup a little leaner.

* Add `.input-group-sm` or `.input-group-lg` to your `.input-group` to
replicate `.input-sm` and `.input-lg`, respectively.
* Add `.btn-group-xs`, `.btn-group-sm`, or `.btn-group-lg` to your
`.btn-group` to replicate `.btn-xs`, `.btn-sm`, and `. btn-lg`,
respectively.

Both sets of new sizing classes simply use the existing individual
sizing classes as mixins, so customization of them is automatically
inherited when compiling.

a2b9988eb9 Media query (grid and responsive utilities) overhaul 
Grid classes have been changed to account for a fourth set of classes,
meaning we now have XS, S, M, and L options. Specifically, we made the
following changes:

* Renames `.col-` to `.col-xs-`
* Adds `.col-md-` for devices 992px wide and up
* Remaps `.col-lg-` for devices 1200px wide and up

Alongside that, we've updated our media queries to better handle
advanced grid layouts:

* All column classes now kick in when they hit a `min-width`, undoing
the #9128 fix in
https://github.com/twbs/bootstrap/commit/db45a60cc86190a060f0baf0b3961cb
c690f3a65
* All column offsets, pushes, and pulls are scoped to an appropriate
range, meaning a `min-width` and `max-width` (thanks for the idea,
@eratzlaff)

We've also modified the widths of our grid containers for the small
device grid (now a max 720px instead of 728px) and large device grid
(now a max 1140px instead of 1170px) to avoid horizontal scrollbar
problems in Firefox (likely due to box-sizing, which is why we didn't
see it in 2.x).

Similarly, we've updated the responsive ultility classes to match the
new four media query approach of the grid system. That means we've:

* Added new `.visible-xs` and `.hidden-xs` classes
* Reassigns visible and hidden classes for small, medium, and large to
matching grid system media queries
* Updates docs examples to match grid system and account for fourth
utility range

ba8026e00f Refactor navbars to simplify things 
* `.navbar-brand` is no longer centered, thus removing need for
max-width and a few lines of code
* Clear floats of `.navbar-brand` in the `.nav-collapse` like we did in
2.x, thus simplifying some clearing and other things.
* Restyle the `.navbar-toggle` button to align it's bars up with the
text and flow of the document.
* Restyle the `.navbar-brand` to not use padding on left/right as to
avoid negative margin and other fuckery. This limits the ability to put
a bg color on the brand, but I think that's fine for now.

a8d95d4721 Improve accessibility (Section 508, WCAG) 
This PR significantly improves Bootstrap's accessibility for users of assistive technology, such as screen readers. Some of the these changes add additional markup to the source examples, but we believe that the sacrifice in readability is worth achieving more widespread usage of accessibility best-practices.

What was done
- Added lots of [WAI-ARIA attributes](http://www.w3.org/WAI/intro/aria)
- Added `.sr-only` helper class, that is only readable by screen readers (and invisible for all other users). This lets us - make progress bars and paginations accessible to screen reading users.
- Advised users to always use label elements. For inline forms, they can hide them with `.sr-only`
- Added 'Skip navigation' link
- Added "Accessibility" section to getting-started.html.

What *wasn't* done
- Contrast issues (twbs#3572)
- Tooltips (twbs#8469)
- Documentation re: usage of icons, since they now live in a separate repo

73c048578d Enable input focus state customization 
- Add new mixin to generate and customize focus state as needed
- Adds variable to set default color
- Include clear disclaimer about customizing this—it's about users', so
don't go making everything bright red and expect them not to be
confused or alarmed.
